Are you looking for a Primary Care Provider (PCP)? FIT offers a full range of Family Practice Services and can be your medical home.
Our Phoenix “Family” Practice clinic is open 5 days a week for your convenience. (we do close many major holidays-call for information)
Sexually Transmitted Disease (STD) testing walk-ins are welcome from 9:00am to 4:30pm Monday – Thursdays and 9:00 am – 1:00pm on Fridays. Fit Healthcare Clinic closes from 1-2pm daily for lunch. Please contact us for holiday testing hours schedule.
Scheduled appointments for medical care are also available during normal business hours. Please Call 602-279-5049 to schedule and if you have any questions or need clarification.
FIT office visits start at $65. For the non-insured, underinsured or those not wanting to use their insurance. We also offer lower cost “cash-pay” labs for those without insurance or who don’t want to use insurance because of high deductibles.
FIT takes many commercial, Medicaid/AHCCCS, Medicare and Market place Insurance plans- please call for information- (PLEASE have your insurance card with ID number and group number available when you call)
In partnership with the public health department, we are excited to offer the following service:
“ASK FOR “THE CORE 4 COUNTY TEST”
$90 for an STD evaluation & testing for Chlamydia, Gonorrhea, HIV and Syphilis. Including medications for the treatment of Gonorrhea, Chlamydia and Syphilis (as needed). This is a test that involves a simple blood draw and urine sample. Results are back in 3 to 7 days.
